CDE Registration Form

This is registration form is used for all CDE's. You must insert the name of the CDE in the blank, as well as your chapter information.

Team members must be currently enrolled in FFA. Enrollment must be for the current school year.

The form can not be sent and will not be accepted if you do not include the names and membership numbers for each of the 4 team members. It will be returned to you. Type "None" in the required spaces if you have less than 4 members. If you have any questions, please contact Regina Holliday (regina.holliday@gmail.com)

Please include t-shirt sizes for all of your team members
in the event that a sponsorship is secured for the shirts.
